Education and Training: I completed 500 hours of classroom training in hypnotism at the Hypnotherapy Academy of America, an accredited school recognized by the University of New Mexico, itself accredited by the United States Department of Education. In addition, I have completed an additional 300 hours of classroom training in pain management, smoking cessation and weight management.
I am a Certified Member and Hypnotist Instructor with the National Guild of Hypnotists, a member of The International Board of Hypnotherapists, The American Council of Hypnotherapist Examiners, and the International Medical and Dental Hypnotherapy Association where I have served as a conference speaker. I do annual training to maintain my memberships in each of these organizations. Additionally, I have an advanced degree as a Naturopathic Doctor (ND) through Trinity School of Natural Health. It is an alternative degree earned through intensive distance learning. In the state of Indiana, NDs are non-licensed professionals. I am a certified fitness trainer through the International Sports Sciences Association, a certified Health Coach through the Institute of Integrative Nutrition, a certified raw food chef/instructor through Living Light Culinary Institute, an accredited California college. I hold additional hypnosis certifications in pain management, smoking cessation, stress management, and weight management.
Notice: AS THE STATE OF INDIANA HAS NOT ADOPTED EDUCATIONAL AND TRAINING STANDARDS FOR THE PRACTICE OF HYPNOTISM, THIS STATEMENT OF CREDENTIALS IS FOR INFORMATIONAL PURPOSES ONLY. Hypnotism is a self-regulating profession and its practitioners are not licensed by state governments. I am not a physician nor a licensed health care provider and may not provide a medical diagnosis nor recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis or any other type of treatment from a different practitioner, the client may seek such services at any time. In the event my services are terminated by a client, the client has a right to coordinated transfer of services to another practitioner. A client has a right to refuse hypnotism services at any time. A client has a right to be free of physical, verbal or sexual abuse. A client has a right to know the expected duration of sessions, and may assert any right without retaliation.
Redress: As a certified member of the National Guild of Hypnotists, I practice in accordance with its Code of Ethics and Standards. If you have a complaint about my services or behavior that I cannot resolve for you personally, you may contact the National Guild of Hypnotists at P.O. Box 308, Merrimack, NH, 03054-0308, (603) 429-9438, to seek redress. Services other than my own may be available to you in the community. You may locate such providers via an Internet search.
Fees: The charge for my service is $200 per private session. I offer a discounted rate of $150 per session if you agree to a contract of four or more private sessions within a six-month period of time. This fee is collected at the time of service. If you choose to pay for all four sessions at the start of your sessions, the fee for service is only $135 per session. My group rate prices are based on package deals. You will receive a written quote prior to the start of your first session.
Notice: If you agree to a contract price, and attend three or fewer sessions within a six-month period of time, you will be liable and your credit card will be charged at the individual session rate of $200 for the sessions you attended. This will result in a higher cost for fewer sessions.
Confidentiality: I will not release any information to anyone without written authorization from you, except as provided for by law. You have a right to be allowed to access my written record about you.
Insurance: I suggest you think of my services as something that you will pay for personally. This both protects your privacy and helps you to value the work you are doing more. In general, insurance companies do not cover hypnotic services, and I caution you not to expect them to do so.
However, some Health Savings Accounts will allow you to use your accrued balance with a prescription for hypnotic services from your doctor. To learn more, contact your HSA provider to fully understand your providers terms of service.
When you register for session(s) you will receive, a copy of your Client Bill of Rights. You are asked to read and sign this form stating you fully understand your rights prior to our first session.
